The post holder will have responsibility for providing specialised interventions for adults with learning disabilities who have complex critical and substantial healthcare needs and meet the eligibility criteria in conjunction with their families and carers.

The post holder will be responsible for the assessment, development, implementation, and evaluation of programmes of care for an identified caseload, ensuring provision for needs-led service. An autonomous practitioner working in partnership and in liaison with the multidisciplinary team, the post holder will uphold the philosophy of individualised client care. Clinical practice will be developed to meet client needs in various settings without supervision.

The post holder may possess specialised skills and knowledge that are utilized and developed in line with local and service needs, as identified within the personal development process.

They will develop their skills and knowledge, aspire to take charge temporarily in the absence of the Team Manager, and provide support and guidance to other team members and agencies.

1. Manage a designated caseload, including liaising with other agencies, service users, and carers.
2. Utilize nursing skills to assess healthcare needs of people with learning disabilities across environments, identify needs, and implement interventions.
3. Evaluate outcomes of nursing interventions to ensure care plan objectives are met and health gains achieved.
4. Ensure interventions are within scope of practice, with training maintained and skills aligned with current guidance.
5. Provide specialist advice and training to carers to improve their caring roles.
6. Undertake additional formal training to qualify as a supplementary or independent non-medical prescriber, contributing to service development.
7. Work collaboratively with colleagues to provide assessment skills and develop comprehensive care plans.
8. Administer and monitor prescribed medication as part of treatment plans, where required.
9. Supervise and guide other nurses and professionals.
10. Support student nurses and other students during placements.
11. Ensure effective communication within the team.
12. Implement risk management strategies to mitigate risks.
13. Contribute to external training supporting therapeutic interventions.
14. Maintain knowledge of relevant legislation such as the Mental Health Act, CTRs, LAEPs, and DSR.
15. Facilitate access to other services, including mental health admissions, and support timely discharges.
16. Advocate for clients and carers, ensuring care needs are met and concerns are escalated.
17. Facilitate access to advocacy services where appropriate.
18. Maintain accurate records in line with regulatory and trust policies.
19. Complete detailed care plans, risk assessments, and treatment programs, involving clients and families.
20. Evaluate care pathways and protocols regularly.
21. Act as a visible role model and foster good working relationships.
22. Provide reports for meetings and contribute to care planning with the MDT.
23. Participate in clinical discussions, supervision, and caseload reviews.
24. Support flexible working practices as needed.
