Overview

Therapy Assistant (fixed-term maternity cover) – Chippenham Community Hospital. 24 hours per week within a multi-disciplinary team in a community hospital inpatient ward. You will support and treatment for patients admitted for a wide variety of reasons, with the primary function of ensuring opportunities for rehabilitation are optimised during their stay on the ward. The post is under the direction of the interdisciplinary team.


Responsibilities

* Assist registered staff in assessment and therapy tasks
* Move, treat and communicate with patients in accordance with the therapy plan
* Implement treatment plans agreed with registered therapy staff, including overseeing and assisting with physical exercise, personal care (e.g. washing, toileting) and practice of activities of daily living, and use of technologies such as computing; support communication and safe swallowing
* Work with patients with cognitive and/or perceptual difficulties as part of rehabilitation
* Train and educate patients, carers and families under the direction of registered staff
* Implement treatment by following care plans and identifying rehabilitation goals
* Keep the patient at the centre of their care, ensuring patient and carer participation in decision making
* Undertake daily rehabilitation and support of delegated patients
* Promote independent function in all daily tasks for patients designated by the registered therapist
* Follow identified OT and PT rehabilitation goals; monitor progress and provide regular feedback
* Support patients to practice mobility and exercise techniques as rehabilitative goals
* Assist ward staff to support rehabilitation goals, referring to registered staff when necessary
* Meet a range of patient and carer needs to reduce length of stay, including clinical tasks relating to physiological function, mental function, or personal and domestic activities of daily living
* Practice principles and philosophy of individualized patient care in accordance with Trust Policy, procedures and regulatory body guidelines
* Obtain consent for any treatment undertaken
* Maintain good knowledge of infection control to reduce risk of healthcare-acquired infections
* Work within a multi-disciplinary team
* Communicate with individuals about routine and daily activities, including overcoming difficulties (e.g. hearing/language)
* Ensure the needs and wishes of the individual are documented
* Deal with distressed patients/clients regularly
* Frequent VDU usage; this may include electronic patient records, making community referrals, discharge forms and accessing equipment websites to organise delivery and collection


Person Specifications

* Essential: Dealing with distressed patients/clients regularly

* Essential: Ability to concentrate on complex/demanding tasks
* Essential: 12 months experience of working in a care setting
* Essential: Some knowledge of general rehabilitation principles
* Essential: Awareness of dementia care
* Essential: Knowledge of and ability to use patient handling and technical equipment relevant to the patient group; knowledge of relevant policies
* Essential: Knowledge and awareness of signs of communication and swallowing disorders
* Essential: Ability to work as a team member; delivering patient-focused care; ability to recognise own limitations
* Essential: Proven ability to motivate and reassure people in difficult or distressing conditions
* Essential: Basic and therapeutic patient moving and handling skills; undertake specific nursing/therapy skills
* Essential: Able to follow routine of the workplace while adapting to unpredictable events in clinical practice
* Desirable: Willing to work in other areas of Wiltshire Health & Care as required
* Desirable: Self-motivated; good time management
* Desirable: Experience of community working
* Desirable: Experience of multidisciplinary team working
* Desirable: Experience in health, physical exercise or rehabilitation environment
* Desirable: Experience providing information, training or education to members of the public
* Desirable: Current full driving license and use of a car
* Desirable: Typing/data input skills
* Other requirements: Willing to work in other areas of Wiltshire Health & Care as needed; self-motivated; good time management
