Role Overview
Support adults living with long-term conditions to live well in the community by working alongside them, their carers and families to understand what matters most and what support is needed to live a safe and independent life.
Responsibilities
* Deliver person‑centred support that empowers individuals to live the life that matters to them.
* Provide a broad range of practice skills, including rights‑based practice, court work, safeguarding, and the development of skills that make a real difference to people’s lives.
* Work flexibly with full‑time, part‑time and hybrid working models to meet the needs of the role.
* Assist with the re‑registration process for those returning to social work.
Qualifications
* Diploma in Social Work (DipSW), BSc/BA Social Work (BSc/BASW) or Masters in Social Work (MSW).
* Social Work Degree Apprenticeship (SWDA) or equivalent professional qualification.
* Valid full UK driving licence and access to a car or efficient public transport travel.
* Current right to work in the UK.
Pay and Progression
Pay ranges from £44,075 for Level 2 and from £48,226 for Level 3 depending on experience. All new starters begin at the entry point of the band, ensuring fairness and consistency across the organisation.
