This is an exciting opportunity for an enthusiastic, compassionate, and highly motivated physical health support worker to work within Bournemouth East Older Persons Community Mental Health Team.
The role involves providing annual physical health checks for individuals diagnosed with a serious mental illness (SMI). This includes conducting physical observations, health promotion interventions, signposting to services, and setting health improvement goals with individuals.
We seek someone committed to improving the physical health of people living with SMI within primary and secondary care, supporting their access to appropriate care.
The candidate must have strong organizational skills, work independently within role boundaries, and seek support from senior colleagues and professionals (GPs, consultants, nurses, VCSE organizations, social prescribers, etc.). The role requires travel across the locality and close collaboration with PCN surgeries.
Applicants requiring current Healthcare worker sponsorship to work in the UK are ineligible.
The post involves working closely with existing teams in primary and secondary care to provide comprehensive physical health assessments (in clinics and homes), signposting, and goal setting to promote self-management and positive lifestyle choices.
You will be managed by Bournemouth East Older Persons Community Mental Health Team but embedded within Primary Care GP Surgeries to meet the physical health needs of individuals in primary and secondary care services.
The role includes developing physical health care plans, fostering supportive relationships with partner organizations, and promoting sustainability.
