An exciting opportunity has arisen for an experienced nurse to join our Urgent Community Response and Reablement team.
The Band 6 Nurse within the Urgent Community Response team plays a pivotal role in delivering rapid, holistic assessments and interventions to individuals at risk of hospital admission.
The aim is to stabilise patients in their own homes, prevent unnecessary admissions, and support timely discharge from acute care settings.
Responsibilities
As a Band 6 Nurse within the Urgent Community Response (UCR) team, you will be responsible for conducting comprehensive, holistic assessments of patients referred to the service and developing tailored care plans in collaboration with multidisciplinary teams.
You will deliver urgent nursing interventions, cohesively working alongside virtual ward colleagues preventing hospital admissions and facilitating safe discharge planning.
The role involves educating and supporting families in basic care delivery, liaising with therapists to arrange necessary equipment and adaptations, and working closely with GPs, social care teams, and acute services to ensure seamless transitions of care.
You will also participate in triage and crisis coordination, maintain accurate clinical records using electronic systems, and communicate effectively with all stakeholders to ensure continuity and safety of care.
Main duties of the job
Your will work as an autonomous practitioner, undertaking comprehensive assessments to identify and meet the holistic needs for individuals under the care of the Urgent Community Response and Reablement team.
You will be responsible for carrying out profession specific and holistic assessments, creating and evaluating care plans as required.
Alongside the multidisciplinary team the successful candidate will plan and develop goal focus rehabilitation/reablement programmes for adults with a wide variety of complex physical and mental health needs.
Qualifications
The ideal candidate will be an experienced registered nurse with knowledge and experience of community services; you will need to be able to prioritise your workload, have excellent communication skills and be able to build therapeutic relations with our partner services such as adult social care and continuing health care.
