Moor Green Lane Medical Centre and Vicarage Road Surgery are recruiting a Senior Health & Wellbeing Coach to join our multidisciplinary primary care team. This is a senior role, suitable for an experienced health coach or public health practitioner who can work autonomously with patients and also contribute to service improvement, project work, and audit activity within general practice. You will support patients with lifestyle and behaviour change, prevention, long-term condition self-management, and wellbeing, while working closely with GPs, nurses, social prescribers, pharmacists, and the wider practice team.
Job Purpose
The Senior Health & Wellbeing Coach is a key member of the practice multidisciplinary team, combining advanced health coaching delivery with public health, prevention, and service improvement activity.The role supports patients to improve health and wellbeing through personalised coaching while also contributing to projects, audits, and service development aligned with practice and PCN priorities.The post-holder will work with a high degree of autonomy and professional judgement.
Key Responsibilities
Advanced Health Coaching
* Accept referrals from GPs, nurses, pharmacists, social prescribers, and the wider practice team.
* Deliver 1:1 health and wellbeing coaching sessions.
* Support patients with:
* lifestyle and behaviour change,
* prevention and early intervention,
* long-term condition self-management,
* low-level mental health and wellbeing needs.
* Co-produce personalised, realistic health and wellbeing plans.
* Provide follow-up to support sustained outcomes.
Projects, Service Improvement & Audit
* Contribute to health improvement and prevention projects within the practices.
* Support service development, quality improvement, and audit activity.
* Assist with data collection, review, and learning to inform service improvement.
* Apply public health and health inequalities principles to service delivery.
Multidisciplinary Working
* Work collaboratively with the MDT to ensure coordinated, patient-centred care.
* Maintain professional boundaries and escalate or refer appropriately when required.
* Share insights and learning with colleagues to support prevention-focused practice.
Data, Governance & Quality
* Maintain accurate and timely clinical records.
* Adhere to safeguarding, confidentiality, information governance, and lone-working policies.
* Participate in supervision, appraisal, and mandatory training.
Person Specification
Essential
* Significant experience in health coaching, public health, wellbeing, or prevention-focused roles.
* Strong understanding of person-centred and behaviour-change approaches.
* Ability to work autonomously and manage complex conversations.
* Experience working with diverse communities and addressing health inequalities.
* Excellent communication and interpersonal skills.
* Sound understanding of safeguarding and professional boundaries.
* Confident IT skills.
Desirable
* Experience in general practice, PCN, or NHS-adjacent settings.
* Public health qualification or equivalent experience.
* Experience contributing to service improvement, projects, or audit.
* Knowledge of lifestyle medicine or prevention approaches.
* Multilingual skills.
