The post holder will be an autonomous practitioner, working within codes of practice, managing a varied and complex caseload to provide highly skilled Occupational Therapy interventions for Monmouthshire residents in their own homes. The goal is to facilitate maintenance and improvement in wellbeing to enable patients to complete daily activities through Reablement and other OT interventions. Peers or external references may provide guidance.
The post holder will collaborate with other Band 6 Occupational Therapists, providing ongoing support, and will work in the central Monmouthshire area alongside multidisciplinary teams in integrated services. They will assess and intervene for patients across Monmouthshire to ensure timely discharge, and will take lead responsibility for tasks such as commissioning and reviewing long-term care and support. The role also involves developing expertise within their clinical specialism.
The key responsibilities include:
1. Practising occupational therapy safely.
2. Planning and prioritising patient workload.
3. Assessing and addressing occupational performance skills in mobility, sensory deficits, intrapersonal and interpersonal skills, self-care, productivity, and leisure.
4. Interpreting patient conditions and providing complex occupational therapy interventions autonomously.
5. Communicating condition-related information empathetically to patients and families, supporting decision-making and goal setting.
6. Identifying and ordering equipment to help patients maintain independence at home.
7. Maintaining records and documentation according to standards.
8. Participating in case conferences and MDT meetings.
9. Adhering to the COT code of conduct and ethics, including confidentiality.
10. Assessing and managing risks.
11. Developing and maintaining collaborative relationships across disciplines and agencies, including employment and retraining agencies.
