Overview
We have an exciting opportunity for a caring and dynamic person to work within the Children's Diabetes Service as a Patient Advocate. The post holder will work alongside the multidisciplinary team in providing high quality patient care for children and young people with both Type 1 and Type 2 Diabetes. The team are based at the Rainbow Community Hub in Elland and care for children in both Huddersfield and Calderdale up to the age of 18.
Responsibilities
* To work holistically in the support of children and young people with diabetes and their families as an integral part of the multidisciplinary team.
* This role often involves working alone in the community visiting children in their homes, in school and other settings; access to own vehicle is essential.
* To support the Consultant, Specialist Nurses, Psychologist and Dietitian in the multidisciplinary clinic, nurse led clinic and transition clinics.
