About the role:
1.Post is a 23 hour per week Support Worker
Working to a six-week rolling rota with the required working hour of 5 weeks at 24 hours and 1 week at 18 hours the rota is subject to change to meet needs of the service.
• There is a requirement to work shifts day and evenings also weekends and Bank Holidays.
You will be supporting Adults who have a Learning Disability to manage their own home as to the principal responsibilities detailed in the Job description.
About you:

What you will do:
• helping people with Learning disabilities to develop their skills and independence by demonstrating and modelling activities.
• Working alongside a dedicated staff team and other professionals in the service users own home and other required venues..
• Undertaking practical tasks and supporting people with their personal care, domestic activities and socially in the community.
• Supporting service users to build and maintain family and friendship links.
• Supporting Service users to reach goals and achieve personal outcomes.
• Participating constructively in regular staff supervision and appraisal sessions.
• Contribute to written assessments and Care Plans and attend Care Planning meetings compiling and keeping detailed records and reports.
• Participating/attending required training to develop, grow and gain skills to enhance the role.
