We are looking for a Heart Failure Nurse who has experience in looking after patients with heart failure to join our established and experienced community service. The post holder will work within a highly skilled team to support and manage patients with complex Heart Failure, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions.

Main duties of the job
1. To Manage a caseload of patients with complex Heart Failure, using evidence -based practice and patient centred goals to assess, plan, implement and evaluate interventions
2. To work as part of the multidisciplinary team with the aim of preventing admission, supporting effective discharge and providing ongoing care for patients with Chronic Heart Failure within the community
3. To provide expert support, advice, and education to raise awareness of the need of the patient with Chronic Heart Failure
4. To Undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
5. The post holder will be an active team participant within the service and provide in-depth clinical knowledge.
