Starting salary for this role is £34,434 (pro rata for part-time colleagues).
Role Purpose
We are based within University Hospitals Dorset (UHD) and work across health and social care services. The team works closely with health partners and community teams. The post is predominantly based at Bournemouth Hospital site but requires support across all acute and community hospitals.
Main Responsibilities
* Undertake comprehensive Care Act assessments focusing on the needs of BCP residents over age 18 and carers.
* Support and enable people to minimise dependence and maximise independence through strength‑based, self‑directed support.
* Work alongside reablement and intermediate care teams and commissioned providers, monitoring and reviewing care packages to meet assessed needs.
* Manage a caseload of BCP residents, visiting hospital settings, care home environments and their homes, and collaborate with hospital staff and other providers.
The Ideal Candidate
* Recent experience working within adult social care or a similar agency with direct contact with the public.
* Understanding of the social and legal context within which social care services are delivered.
* Excellent communication skills, able to advocate for adults, signpost and provide information and advice.
