Role Purpose
The NFO Assertive Outreach Support Worker delivers the Near Fatal Overdose (NFO) response service across Fife, providing timely, person-centred follow-up to adults who have experienced a near-fatal overdose. The role supports harm reduction, recovery engagement, and reducing repeat overdose risk.
The service operates within a shared care model alongside the Scottish Ambulance Service, Fife ADP, and treatment providers. The postholder will work with individuals and, where appropriate, families or significant others to address health, social, and recovery needs.
Key Responsibilities
· Provide assertive outreach and follow-up after naloxone-related call-outs.· Deliver person-centred case management and recovery planning.
· Support access to treatment and community services.· Provide crisis and relapse prevention support.· Work in line with national Quality Principles.
Interventions & Harm Reduction
· Develop recovery plans collaboratively.· Work with families where appropriate.· Support triage into OST and treatment pathways.· Provide naloxone training and distribution.· Deliver harm reduction advice and brief interventions.
· Support DBST/BBV and sexual health testing.· Maintain accurate records.· Liaise with professionals.
CALM Support
· Provide intensive engagement for individuals with complex needs.
Data & Reporting
· Submit triage assessments via FORT.· Support 90%+ treatment entry within 21 days target.· Maintain GDPR-compliant records.
Partnership Working
· Liaise with NHS Fife, GPs, social work and partners.
Professional Practice
· Participate in supervision and appraisal.
· Maintain CPD.· Work flexibly including out-of-hours.· Undertake other duties as required.
Person Specification
Qualifications· SVQ Level II/III in Social Care or equivalent.· COSCA Counselling Skills or similar.· Relevant qualification in health, social care, or community work.
Experience· Experience supporting individuals with complex needs.· Experience in health, social care, or community settings.
Desirable:
· Experience in substance use or harm reduction services.· Knowledge of naloxone, BBV testing, or recovery services.
Skills & Competencies
· Empathetic and non-judgemental approach.· Strong communication and partnership skills.· Good organisation and time management.· Ability to work independently and in a team.· Competent IT and record-keeping skills.· Ability to travel across Fife.· Full UK driving licence.
Desirable:
· Motivational Interviewing skills.· Experience using case management systems such as FORT.
