About the Position
The Integrated Discharge Team (IDT) is looking for a motivated and dynamic Band 5 Occupational Therapist to join our team. This post offers a chance to develop clinical and leadership skills within a supportive and innovative environment, with the opportunity for a preceptorship period if not already completed.
IDT is a valued part of UHCW flow and the Integrated Care and Community Services group, continually evolving to meet patient needs. We celebrate team success while prioritising the wellbeing of all team members, offering regular supervisions to support this.
The service operates 7 days a week with extended hours (08:00‑18:00 Monday‑Friday). We welcome applications from Occupational Therapists currently practising, returning to practice or newly qualified with an interest in discharge.
Key Responsibilities
* Provide assessments, coordinate and refer patients for support along their recovery journey, maintaining and restoring functional abilities while considering individual needs and environments.
* Manage a caseload, prioritise assessments, and demonstrate clear clinical reasoning and evidence‑based practice with support from the team.
* Actively promote the role of Occupational Therapy and the IDT Service.
* Provide clinical support to less-experienced staff as delegated by seniors.
* Work as an autonomous member of the multidisciplinary team, contributing to discussions and influencing decisions about patient care and discharge pathways.
* Represent IDT at board rounds, proactively driving discharge, questioning plans, predicted dates and escalating delays to managers.
* Hold responsibility for own caseload assessing health and social care needs, ensuring correct support levels for discharge.
* Review patients on active caseload and amend discharge plans as required, seeking senior support for tasks outside identified competency.
* Carry out assessments using a variety of modalities, including physical, cognitive, perceptual and psychological interventions, recording outcomes and liaising with community services to support discharges.
* Identify and order equipment required to support discharge for patients case‑managed by IDT.
* Effectively communicate the discharge process and plans to patients, families and carers, leading on discharge planning meetings as required.
Qualifications & Experience
Applicants must be qualified Occupational Therapists with a valid HCPC registration, currently practising or returning to practice. Experience in discharge planning, multidisciplinary teamwork and evidence‑based practice will be an advantage.
