Overview
Are you an experienced Learning Disabilities Case Manager with a strong background in Continuing Healthcare (CHC)? We are working with an NHS organisation in Yorkshire seeking a skilled interim professional to join their team for an initial 3-month contract. This role offers hybrid working, with 1 day on site per week.
Responsibilities
* Manage a caseload of individuals with Learning Disabilities eligible for NHS Continuing Healthcare (CHC) funding
* Complete CHC assessments, reviews, and eligibility panels in line with National Framework guidelines
* Work collaboratively with multi-disciplinary teams and external providers to ensure safe and person-centred care
* Provide expert input to complex case reviews, including joint funding and placement decisions
* Maintain accurate and up-to-date records, ensuring compliance with national standards and local procedures
