Are you a skilled and compassionate Adults Social Worker ready to take the next step in your career? Join a committed First Contact Team in the North East and play a crucial role at the front door of adult social care, where timely intervention makes all the difference.
The Role
Based in Stockton Borough Council’s First Contact Team, you’ll be the first point of contact for adult safeguarding concerns and complex referrals. You’ll triage incoming information, assess risks, and ensure that appropriate decisions are made in line with legislation and guidance.
While the team is based at Dunedin House, Stockton, there’s flexibility for remote working once you’re fully trained. Expect a supportive and collaborative team environment, with regular opportunities to be office-based for team meetings and joint decision-making.
Key Responsibilities
* Triage and manage adult safeguarding referrals
* Make clear, evidence-based decisions using legislation and local guidance
* Conduct Care Act Assessments in both community and hospital settings
* Undertake Mental Capacity Assessments and Best Interest decisions
* Develop tailored, person-centred support plans
* Liaise with the Integrated Hospital Discharge Team at Scunthorpe General Hospital
* Support unqualified staff and mentor student social workers
About You
We’re looking for someone who is:
A Qualified Social Worker registered with Social Work England
Experienced in Adult Social Care (3+ years preferred)
Skilled in adult safeguarding, risk assessment, and multi-agency working
Knowledgeable in Care Act 2014, Mental Capacity Act 2005, and Human Rights Act
An excellent communicator with strong decision-making and reflective skills
Comfortable working both independently and collaboratively
