Attend and actively participate in all team meetings. To administer prescribed medicines to patients either in a clinic or out of hospital setting, predominately in the patients home: To ensure all patient care and administration of prescribed medicines are provided to first class clinical and ethical standards and demonstrates best practice. To provide education for patients / carers in the out of hospital environment as required. To provide support and advice to patients, as appropriate.
To develop string working relationships with patients clinical teams within the secondary and primary healthcare settings, as appropriate. To manage the interface between hospital services and other agencies in order to meet patients needs. To have the ability and willingness to travel to meet the needs of our patient cohort. To ensure efficient and appropriate record keeping in relation to patient treatment: To maintain accurate records of care and completion of these in a timely manner To assist in the recording / reporting of adverse events / incidents, including side-effects of therapy To report to and work closely with your manager, providing all appropriate communication in an agreed and timely manner.
To demonstrate excellent time management skills, providing a flexible service that meets patients needs. To maintain self and patient safety through the appropriate use of policies, protocols, and guidelines. Ability to work alone and within a team with confidence. To support and mentor new nurse team colleagues and provide feedback to the regional nurse managers. To undertake appropriate training, i.e., SACT Passport, and participation in clinical trials work, where required.
