Urgent Care Response Team Occupational Therapist/Physiotherapist

To lead in the delivery of high standards of Physiotherapy/Occupational therapist interventions to patients within their own place of residence to prevent avoidable admissions to acute care and to support discharges. Post holders will provide short term reablement/rehabilitation programmes with effective short term goal setting. This will involve using health coaching skills to effectively engage with the patient and their family to ensure expectations are appropriately managed. The role requires flexibility, working across the following services: Urgent Care Coordination Hub/NEAT, Discharge to Access, and Virtual ward to form part of the Urgent Care Response Team.

Main duties of the job
As part of the Urgent Community Response (UCR), you will be required:
1. To work within the Urgent Community Response Team to prevent unnecessary admission to hospital and support hospital discharges through the Discharge to Assess pathway. This service requires 7-day working, with weekends covered on a rota basis.
2. To make risk assessments, establishing the safety or otherwise of a patient's ability to stay at home or return safely from another care setting, providing and arranging supportive packages of care as necessary.
3. To work within the team to assess and prescribe care pathways for patients in crisis, including those with long-term conditions, frailty, palliative and end-of-life care, and rehabilitation needs, aiming to improve quality of life and independence.
4. To act as the patient's advocate, facilitating choice and patient empowerment.
5. To provide evidence-based clinical/therapeutic interventions based on 'best practice' to improve health outcomes and promote choice.
6. To provide in-reach to inpatient units and step-up beds to support early discharge from hospital.
7. To collaborate with healthcare professionals and statutory/non-statutory agencies to ensure seamless, integrated services.
8. To triage referrals on a rotational basis for the Urgent Community Response Team.

Job responsibilities
The post holder will:
1. Undertake comprehensive physiotherapy/occupational assessments, including complex cases, using investigative and clinical reasoning skills, and inform differential diagnoses.
2. Develop and implement individualized management and treatment plans in collaboration with the team, patients, and other agencies.
3. Be aware of assistive technology and refer as required.
4. Assess holistic needs, communicating complex information effectively to patients and carers, adjusting explanations for comprehension barriers.
5. Establish and maintain therapeutic relationships, motivating and encouraging patients.
6. Liaise with GPs, social services, inpatient teams, and voluntary sectors for coordinated care.
7. Participate in multi-disciplinary meetings, e.g., Gold Standard Framework.
8. Ensure activity data and clinical information are recorded accurately on Systm One daily.
9. Plan and organize specialist services within the team.
10. Demonstrate manual dexterity and skills for assessment and treatment, including using specialist equipment.
11. Order equipment and supplies via electronic systems.
12. Obtain informed consent before interventions.
13. Adhere to infection control policies, including universal precautions.

* Current HCPC registration
