The role
We are looking for a Support Worker to join our Single Homeless Pathways team at Southern Housing!
In this role, you will work as part of a team across multiple locations providing support services to vulnerable residents, promoting independence within their home and the wider community. You will support daily living, social skills, health, and wellbeing to individuals who are young or have complex needs, helping them develop independent living skills, remain independent, and prevent future homelessness.
This is a full-time position based at various schemes around the Island in Newport, Sandown, Shanklin, and Ryde.
Please note, this is a 24/7 service, and you will be required to undertake sleep-in duties (if applicable) and may be on the on-call rota. A full UK driver’s license is required.
This is a Fixed Term Contract until 31-May-2026.
What you’ll be doing:
* Providing support to residents based on their needs assessment and support plan, following the principles of housing first and psychologically informed environments
* Delivering services that promote independence, rights, choices, dignity, and inclusion
* Practicing within local authority criteria, ensuring support plans are current and reflect residents' needs and wishes
* Conducting key working sessions, developing engagement, and creating plans to help residents achieve their goals
* Maintaining professional relationships with residents and stakeholders
* Promoting life and social skills and encouraging resident involvement
* Signposting and facilitating access to specialist services as needed
* Supporting residents with home security and maintaining a safe living environment
* Adhering to health and safety policies, including lone working and infection control
What you'll have:
* Strong written and verbal communication skills
* Empathy, compassion, and patience
* Adaptability to different working environments
* Willingness to train towards an NVQ 2 in Care
* At least 5 GCSEs at grade C or above, including Maths and English
* Willingness to pursue NVQ 3 or equivalent
* Ability to acquire necessary underpinning knowledge for the role
