The post holder will provide a programme of social, creative and skills-based activities to both individuals and groups under supervision. The post holder will be responsible for leading the activities daily in coordination with the occupational therapist and nursing/support staff. The post-holder will work collaboratively with individuals to promote their recovery and enhance social inclusion.
Duties and Responsibilities;
Clinical
1. Work as a team member in delivering and evaluating clinical interventions using activities to help clients achieve, develop, or maintain daily living skills, involving both individual work and group facilitation.
2. Assist in implementing joint care plans (MDT) focusing on physical, social, and mental health needs.
3. Support patients in engaging in occupational, diversionary, and recreational activities, using occupational skills and equipment, including preparation and organization for group activities.
4. Record all interventions, observations, and responses in Patient MDT records.
5. Escort patients to therapeutic/treatment programmes as per care plans and MDT instructions, within hospital or community settings.
6. Develop knowledge of local resources to support patient engagement in community activities.
7. Support and assist patients in managing daily living skills to promote personal recovery, recognizing varying levels of assistance.
8. Participate in planning and executing community outings, ensuring safety and social inclusion, considering risk and escorting status.
9. Participate in regular audits to evaluate patient participation and group program effectiveness.
10. Respect patient rights, promote dignity and self-esteem, and maintain appropriate boundaries.
11. Contribute to risk assessments and manage risks effectively in accordance with policies.
12. Ensure culturally sensitive, relevant, and accessible care services.
13. Promote informed decision-making through honest communication with patients and carers.
14. Collaborate with occupational therapists and nursing staff in daily tasks and delegated responsibilities, including equipment checks and resource management.
15. Exercise good personal time management and ensure reliable attendance.
16. Report maintenance issues and collaborate on service improvements.
Professional Development
1. Participate in supervision and training activities to support practice.
2. Assist in developing and delivering training under supervision.
3. Engage in CPD activities as discussed with the Occupational Therapist.
Communication
1. Respect confidentiality and communicate professionally.
2. Maintain accurate clinical records and statistics.
3. Gain consent and build collaborative relationships with patients to promote engagement.
4. Encourage patients to access their recovery plans.
5. Respect team members and demonstrate professional respect.
Service Development and Delivery
1. Adhere to health and safety guidelines and ensure safe use of equipment.
2. Report health and safety issues to senior staff.
