Band 6 Senior Community Mental Health Nurse/Occupational Therapist/Social Worker
We are excited to announce this position within our Adult Community Mental Health Team covering West Norfolk. This is a fantastic opportunity for practitioners motivated to advance their career path. If you are currently in a Band 5/6 position and have at least 12 months post‑registration experience, this may be the post for you. We welcome applications from nurses, social workers and occupational therapists. We value the unique skills that all of the different professions bring to the team and will support you to utilise your specialist skills, attend profession‑specific training and support your CPD.
Responsibilities
* Managing a caseload and providing mental health care services to people with mental health care need.
* Provide planned care, risk assessment and screening for patients within their care plan and ongoing evaluation of patients’ needs and identify suitability for potential discharge in conjunction with the CTL, via caseload management/supervision.
* Initiate and attend patient‑centred reviews, case conferences and other meetings as required.
* Advise the Responsible Medical Officer(s) and/or health and social care professionals of any change in the patients’ conditions and/or circumstances.
* Work within the MDT and form working partnerships with other care providers.
* Ensure service users/carers are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the MDT.
* Provide caseload management supervision to junior staff under the direction and supervision of the CTL.
* Assist with the supervision of peers if requested.
* Enable patients to gain and maintain independence.
* Identify personal learning and training needs through KSF, appraisal and personal development plans on an annual basis.
* Undertake supervision of students undertaking placements within the Team.
