Job Purpose
Claimont Home Care Ltd is a CQC registered private sector service delivering specialist mental health nursing at home when inpatient admission is being considered or for inpatients treated in hospital but who wish to be discharged home. We invest in people and build relationships based on mutual support and long term commitment. The work is rooted in a Patient Centred, holistic approach. We are looking for high quality, dynamic and experienced Mental Health Support Workers who can provide waking night support and/or day support to join our team of Bank Staff.
Key Responsibilities
* Carry out client care on a home visit, focusing on one-to-one person-centred support.
* Support documentation and record-keeping for all aspects of client treatment and progress (including electronic records within 24 hours of events).
* Complete and maintain all required documentation and records; ensure confidentiality and data protection.
* Actively participate in team meetings; contribute to care planning and delivery.
* Provide activities with clients that enable full access to community life; support families/carers in understanding and coping with mental health issues.
* Maintain professional core psychiatric skills and develop links with community services/resources.
* Adhere to policies, protocols, and national/local guidance; ensure safe practice and safeguarding.
* Attend mandatory training, health and safety requirements, and personal development activities.
* Observe equal opportunities policy and promote health and safety within the team and to service users.
Responsibilities to Stakeholders
* Maintain effective communication with referring agents, colleagues, carers, and external professionals.
* Ensure health, safety, and wellbeing of patients remains central to all activities.
* Support information flow within the team and with external parties.
Job Requirements
Experience:
* Essential: Minimum 12 months mental health (PTSD & Personality Disorder) experience; experience in a multi-professional team; ability to work collaboratively; ability to build trust with patients.
* Desirable: Experience delivering health care services to people with mental health problems across settings (including community, forensic).
Qualifications:
* Essential: Basic computer skills.
* Desirable: Post-basic qualifications in health care; degree-level post-basic qualifications; current, clean, full driving licence.
Skills:
* Excellent verbal and written communication; excellent report writing; strong interpersonal skills.
* Ability to manage change, work to deadlines, be flexible, and work collaboratively.
* Ability to self-manage; crisis management; ability to build relationships in difficult situations.
* Knowledge of health care delivery within NHS/Independent sector; understanding of Mental Health Act 1983 and Health and Social Care Act implications.
