Job Overview:
Assist street outreach workers in providing support to rough sleepers. Engage with clients, assess their needs, and help them access accommodation and services.
Key Responsibilities:
* Conduct outreach Monday-Friday
* Collaborate with agencies to provide housing and support options
* Advocate for clients' welfare and healthcare needs
* Manage a caseload, offering guidance until clients are housed
* Maintain accurate case notes on CHAIN and other databases
