Job overview

Redbridge, North East London Foundation Trust is seeking an enthusiastic, motivated Registered Nurse with a minimum of four years’ primary care experience to join the Integrated Care Team as a Band 7 District Nurse Team Leader. The successful candidate must have proven experience working as a District Nurse Team Leader.

Key responsibilities

1. Lead and manage a District Nursing team within the Integrated Care Team.
2. Provide high‑quality clinical care and supervision across community settings.
3. Oversee caseload allocation, governance, and performance management.
4. Promote service development, quality improvement, and partnership working with primary, secondary, and social care colleagues.
5. Support education, training, and clinical competence of team members.
6. Ensure robust record keeping and use of IT systems for care documentation and reporting.
7. Contribute to end-of-life care planning and delivery.

Main duties of the job

8. Deliver safe care to a caseload of patients allocated to the team and be professionally and legally accountable for care provided.
9. Lead and manage a District Nursing team within the Integrated Care Team, line manage Community Nurses and Health Care Support Workers and supervise and appraise staff members and students.
10. Work alongside the Community Matron and Deputy Clinical Nurse Lead to support integrated service delivery.
11. Provide high‑quality clinical care and supervision across community settings.
12. Oversee caseload allocation, governance, and performance management.
13. Promote service development, quality improvement, and partnership working with primary, secondary, and social care colleagues.
14. Support education, training, and clinical competence of team members.
15. Ensure robust record keeping and effective use of IT systems for care documentation and reporting.
16. Contribute to end-of-life care planning and delivery.
17. Hold a valid UK driving Licence

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,714 and a maximum payment of £5,941) pro rata for part time staff.
