Job Overview
A qualified clinician is required to deliver high‑risk podiatric care for patients with complex diabetic foot problems across community clinics and the multidisciplinary foot team at Hillingdon Hospital.
The role supports multidisciplinary foot and vascular services, delivers education to staff and patients and works independently as well as part of a team.
Responsibilities
* Work in accordance with the College of Podiatry and HCPC Code of Professional Conduct.
* Assess, manage and treat high‑risk complex caseload including patients with acute and chronic diabetic foot problems, pre and post‑operative amputations, neuropathic and vascular foot/lower limb problems.
* Lead planning of care packages jointly between hospital and community care providers.
* Provide podiatry and wound care for patients with musculoskeletal conditions.
* Undertake biomechanical assessment, gait analysis and orthotic provision for a range of underlying MSK/biomechanical conditions.
* Use clinical reasoning to assess, diagnose and formulate treatment plans for high‑risk foot patients.
* Adopt unpredictable work patterns for acute wound care clinics.
* Apply high‑skill debridement techniques for acute/chronic, neuropathic and ischaemic wounds.
* Contribute to care and management of complex wounds and lower‑limb complications.
* Practice principles of wound management, including dressing selection and indications.
* Interpret investigations such as microbiology, X‑ray and scans and respond appropriately.
* Act as the clinical diabetes / high‑risk foot specialist within the multidisciplinary foot team.
* Promote multidisciplinary team working.
* Provide diabetes / high‑risk foot care to hospital out‑patients and wards based on agreed referral criteria.
* Accept direct referrals from GPs, community podiatry and nursing for rapid access and expert intervention.
* Take internal hospital referrals from consultants/doctors, nurses and A&E based on agreed criteria.
* Undertake specialist high‑risk work in acute and community settings.
* Perform nail surgery where indicated.
Qualifications
Essential criteria
* Diploma/BSc (Hons) Podiatry
* Certificate for Local Anaesthesia
* Registered Member of the HCPC
* RCOP Diabetic Foot module or equivalent
Desirable criteria
* IRMER
* Clinical masters degree
* Woundcare Course
* Independent Prescriber status
Experience
Essential criteria
* Extensive postgraduate experience in NHS high‑risk diabetic foot podiatry.
* Experience of educating healthcare professionals.
* Wide range of clinical experience, including complex wound care with emphasis on diabetes patients.
* Experience of multidisciplinary working.
* Experience of working across primary and secondary care.
Desirable criteria
* Experience delivering patient education packages.
Skills and Knowledge
Essential criteria
* Extensive knowledge of the effect of diabetes in podiatric care.
* Sound knowledge and experience in wound care, treatment modalities and biomechanics.
* Ability to demonstrate skills in off‑loading for high‑risk foot ulceration prevention.
* Current clinical knowledge regarding evidence base and best practices.
* Ability to order appropriate investigations for high‑risk foot treatment.
* Experience of foot health education and promotion.
* Knowledge of national policy relating to the diabetic foot.
* Effective organisational skills.
* Knowledge of issues involved with diverse populations.
Key Skills and Abilities
Essential criteria
* Excellent communication, IT, negotiation and interpersonal skills.
* Ability to prioritise and plan own workload to meet deadlines and competing demands while demonstrating flexibility.
* Involvement in regular audit or R&D.

Essential criteria
* Must be able to undertake domiciliary work and travel between clinics in the Hillingdon area (car driver/owner).
