Young People Leaving Care Support Worker – Doncaster
Outreach – hours are between 9 and 5, between 20-30 hours a week, flexible and ongoing.
This is a long-term temporary position; however, there is no end date if you are subscribed to the update service and have a short notice period. A quick start is possible through a temp-to-perm placement.
£15 + 40p mileage per hour.

Hours are between 9-5, Monday to Friday. Flexibility is required to meet the needs of the young people, but the position also offers flexibility for the worker.
Our client is a leading provider of care to children and young people in the area, currently recruiting for a Support Worker on a permanent basis.
A quick start is possible if you have a current online DBS.
This role involves supporting families and vulnerable young people.
It is essential that you have experience working within a similar setting and preferably have completed NVQ Level 3 Children & Young People Workforce or a similar qualification within this sector.
Qualifications/Requirements
High standard of general education.
Preferable: NVQ Level 3 Health & Social Care – Children and Young People (CCYP) or equivalent.
Willingness to undertake further training.
Job Purpose
To be responsible for providing support to vulnerable people (service users) for the company.
Main Duties & Responsibilities
1. Assist the young person’s allocated social worker in delivering the service.
1. Provide transport/escort to the young person as directed in line with care plans.
2. Conduct welcome and induction for young people.
3. Perform ongoing assessments of service users’ needs.
4. Provide ongoing support and monitoring of service users.
5. Assist with welfare and NHS issues.
6. Prepare reports.
7. Arrange appointments with partner agencies.
8. Maintain regular contact with service users.
9. Inform service users of support groups.
10. Assist with move-on processes for service users.
2. Accurately record, create, maintain, and update electronic records of service users.
3. Prepare for and attend meetings as required.
4. Provide mediation to service users as required or directed.
5. Use IT packages effectively to support work.
6. Work cooperatively across multidisciplinary teams.
7. Maintain strict confidentiality regarding service users, clients, and the company.
8. Assist in the development of company policies and procedures.
9. Report housing issues to the services department and assist in resolving defects.
10. Undertake on-call duties as directed.
11. Participate in office reception duties as required.
