Job Summary
An experienced Clinical Pharmacist qualified in the CPPE practice pathway (18‑month training) and an independent prescriber is sought for Sidcup Medical Centre. The role is part of a multi‑disciplinary team working at Sidcup Medical Centre, Sidcup, Kent.
Responsibilities
Key duties include:
* Lead structured medication reviews, optimise prescribing, reduce waste, and promote self‑care.
* Support patients with long‑term conditions, signpost to community pharmacy and refer to GPs where appropriate.
* Provide patient‑facing clinics, virtual advice and follow‑up for medication optimisation.
* Liaise with BHNC pharmacists and other practice pharmacists on prescribing matters to ensure patient care consistency.
* Reconcile medicines after discharge from hospitals, intermediate care and into care homes, ensuring continuity of care.
* Produce accurate, contemporaneous records of patient consultations in line with legislation and policy.
* Maintain budget management, identify efficiencies, and manage resources required for key duties.
Qualifications – Essential
* General Practice Pharmacist Training Pathway from CPPE or equivalent qualification, and exemption agreed by CPPE.
* Registration with the General Pharmaceutical Council (MPharm) or equivalent qualification.
* Membership of the Royal Pharmaceutical Society.
* Completed Independent Prescriber course.
* Basic life support training and safeguarding mandatory training.
Qualifications – Desirable
* In‑depth therapeutic and clinical knowledge and understanding of evidence‑based healthcare.
* Excellent interpersonal, influencing and negotiation skills.
Experience
* Minimum of 2 years experience as a pharmacist in a practice portfolio.
* Experience of partnership/collaborative working and building relationships across a variety of organisations.
Person Specification – Essential Skills
* Analysing complex technical data and information.
* Identifying and resolving risk‑management issues according to policy/protocol.
* Excellent written and verbal communication, influencing and negotiation skills.
