To work as a frailty specialist in the A&E department. Carrying out frailty assessments and finding opportunities for alternative safe options for patient care in the community (admission avoidance).
This is a small, highly specialised team working at the centre of the A&E department. It is an opportunity to develop your skills and experience as a Frailty Specialist.
You will have previous experience and knowledge of frailty assessment and some understanding of A&E processes. You will contribute to the MDT and have confidence to provide challenge in relation to best options for care and opportunities for admission avoidance.
Main duties of the job
Screen and triage older people with frailty who attend the Emergency Department (ED) in order to: prevent avoidable admissions, reduce time waiting to be seen, optimise the quality of care in the ED and ensure patients are discharged home as soon as medically optimised.
In doing so liaise with appropriate services ensuring patient safety at all times. The role is expected to develop and help shape a new operating framework for the management of Frail Patients within the Trust.
This post will:
Work as part of an intradisciplinary team initiating Comprehensive Geriatric Assessment
Develop and embed the use of frailty scoring within the organisation.
Analyse frailty scores and develop a strategy that ensures robust pathways of care.
As an autonomous practitioner, the post holder will be responsible for managing a short-term patient caseload ensuring that effective and robust clinical interventions are implemented.
Link with appropriate stakeholders both within and external to the pathway development.
