This role is an exciting opportunity to join our team within the Havant & Waterlooville PCN who are collectively responsible for delivering high quality patient services to 68000 patients.

Pharmacy technicians play an important role in complementing clinical pharmacists and other members of the PCN team as well as staff within each practice and external organisations.

The purpose of the role is to improve patient care by implementing best practices and maximising prescribing safety. The post holder will help patients get the most from their medicines, improve repeat prescribing processes, lead and promote repeat dispensing/online ordering as well as aiming to reduce medicine waste, as well as assisting the four practices across the PCN with on the-day queries.

In addition, the post holder will be responsible for developing the understanding of medicines optimisation throughout the practice teams. This will involve assisting the PCN in achieving national requirements, NICE implementation and the utilisation of medicines optimisation initiatives.

As well as the PCN national requirements you will assist all of the PCN practices four with their practice-based needs, assisting practice prescribing teams with inquiries.


Main duties of the job

Undertake patient-facing activities to ensureeffective medicines use through shared decision-making discussions withpatients.Carry out:

medicines optimisation tasks includingeffective medicines administration (eg checking inhaler techniques)

patient safety audits and incentive schemeactions, as directed

Supportmedication reviews and medicines reconciliation. Where required, utiliseconsultation skills to work in partnership with patient to ensure they usetheir medicines effectively

Synchronise medicines for patients transferringbetween care settings and link them with local community pharmacists asappropriate

Provide specialist expertise, where competent,to address both the public health and social care needs of patients, includinglifestyle advice, service information, and help in tackling health inequalities

Support clinical pharmacists with shared careprotocols

Support initiatives for antimicrobialstewardship to reduce inappropriate antibiotic prescribing

Support the implementation of national andlocal prescribing policies and guidance within GP practices, care homes andother primary care settings this will be achieved through undertakingclinical audits, supporting delivery of QIPP, QOF and enhanced service (LCS andIIF) targets

Havant and Waterlooville Primary Care Network (PCN) has five member practices* in an area to the south of Hampshire, stretching from Denmead Practice in the north-west to Bosmere Medical Practice to the south of Havant. Together our practices are responsible for 68,000 patients

You will work partly from within a primary care setting and partly in the community.

The post-holder will have the confidence to work with clinical and non-clinical colleagues from Havant & Waterlooville PCN practices, PCN colleagues, including Social Prescribers, Health & Wellbeing Coaches, Pharmacists, First Contact MSK Practitioners and Mental Health Practitioners, Care Co-ordinators and colleagues working in secondary care, community health, adult social care and the voluntary sector.

Clinical responsibilities

Support medication reviews and medicines reconciliation, including for new care home patients. Where required, utilise consultation skills to work in partnership with patient to ensure they use their medicines effectively

Synchronise medicines for patients transferring between care settings and link them with local community pharmacists as appropriate

Provide specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling health inequalities

Support clinical pharmacists with shared care protocols

Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing

Support the implementation of national and local prescribing policies and guidance within GP practices, care homes and other primary care settings this will be achieved through undertaking clinical audits, supporting delivery of QIPP, QOF and enhanced service (LCS and IIF) targets

Technical and Administrative responsibilities

Understand and use EMIS clinical record system effectively

Develop and implement schemes and action plans for delivery of PCN-wide Investment & Impact Fund (IIF) scheme targets.

Work with PCN MDTs and practices to support medicines optimisation in order to get the best value from medicines/reduce wastage

First port of call for practice reception teams triaging general prescription requests, to allow GPs and Clinical Pharmacists to focus on more clinically complex requests

Encourage uptake and implementation of Electronic Prescriptions, safe repeat prescribing systems, and timely monitoring and management of high-risk medicines

Develop relationships with other pharmacy technicians, pharmacists and members of PCN MDTs to support integration of the pharmacy team with other PCN and practice teams and with community pharmacy and secondary care colleagues

Assist in delivery of medicines optimisation incentive schemes (eg medicines switches/alternatives) and ensure that all activities are monitored and evaluated

Attend meetings as required; attend training courses and supervision sessions as required; keep up to date with Continuing Professional Development (CPD)

Work in accordance with PCN policies, including the equal opportunities policy and practice

Undertake any other duties as may reasonably be required from time to time

Be prepared to work flexibly, including evenings and weekends if required

Person Specification


Qualifications

* Current registration with the GPhC
* Evidence of CPD throughout employment
* Excellent communication & interpersonal skills
* Numeracy and literacy skills
* Ability to work in a team and alone as required
* Trustworthy, enthusiastic, and self-motivated
* Full driving licence
* Enrolled in, undertaking or qualified from PCEP or MOCH
* Experience of:
* working in primary care
* EMIS clinical record system
* Patient counselling and consultation skill
* Time management and prioritisation skill
* Logical reasoning/problem solving skill
* Adaptive, innovative and forward thinking
* Good clinical knowledge of common long-term conditions and clinical terminologies
* Clinical audit knowledge and experience
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
