Offer description

We are looking for a dedicated and proactive Paramedic, who is passionate about high-quality patient care to join our friendly multi-disciplinary team.

As part of the Purley & Coulsdon PCN, you will work closely with our GPs, nurses, ANPs, physiotherapists and administrative teams to deliver safe, effective, and person-centred care across our population of just under 18,000 patients.

This post is a part-time role for 20 hours per week.


Main duties of the job

As a Paramedic, you will be a valued member of the clinical team, responsible for:


Clinical Work

* Telephone and face-to-face consultations for minor illness/injury
* Clinical assessment, diagnosis, treatment and referral
* Community and care-home visits
* Advising patients on health and wellbeing
* Maintaining accurate clinical records
* Supporting audits and service improvements


Operational & Collaborative Work

* Working within own professional scope and code of conduct (HCPC and College of Paramedics)
* Supporting PCN quality and outcomes framework (QOF) and other targets
* Contributing to audits, service improvement, and implementation of local and national guidelines


Job responsibilities

* Workas part of a multi-disciplinary team (MDT)
* Assessand triage patients, including same day triage and as appropriate, provide definitive treatment, following patient group directives, NICE (national), local clinical guidelines and local care pathways and make necessary referralsto other members of the primary care team
* Advisepatients on general healthcare and promote self-management where appropriate,including signposting patients to the organisations social prescribing serviceand, where appropriate, other community or voluntary services
* Beable to -
* Performspecialist health checks and reviews within their scope of practice and in line withlocal and national guidance
* Performand interpret ECGs
* Performinvestigatory procedures as required and undertake the collection ofpathological specimens including intravenous blood samples, swabs and othersamples within their scope of practice and within line of local and nationalguidance
* Supportthe delivery of anticipatory care plans and lead certain services (e.g.learning disability health checks, SMIs and diabetic reviews of care home andhousebound patients)
* Providean alternative model to urgent and same day GP home visit for the practice
* Communicateat all levels across organisations ensuring that an effective, person-centredservice is delivered
* Communicateproactively and effectively with all colleagues across the MDT, attending andcontributing to meetings as required
* Maintainaccurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient healthcare and registration with the organisation
* Communicate effectively with patients and, whereappropriate, family members and their carers, where applicable, complex andsensitive information regarding their physical health needs, results, findingsand treatment choices


Skills & Abilities

* Ability to manage and prioritise own workload
* Ability to work autonomously, using own clinical judgment to form a diagnosis
* Excellent communication skills and be able to function effectively as part of a team
* A proactive, compassionate approach and a commitment to improving patient outcomes.


Experience

* Experience as an autonomous practitioner within a minor injury, treatment centre or mobile service
* Experience of working in primary care/general practice
* Experience of using EMIS


Qualifications

* Registered with HCPC
* Pre-reg MSc or working towards ACP status
