We are a team of Occupational Therapists, Physiotherapists, and advanced clinical practitioners working closely with the multidisciplinary team to prevent further deterioration in patients' physical functioning and wellbeing. We cover the areas of Chorley, Preston, and South Ribble.
Your role will be to support the team, including undertaking non-complex assessments and carrying out treatment plans under the direction of the therapists.

The role involves working with patients in their own homes and various community settings. The service operates 365 days a year, from 8.00 am to 8.00 pm.
