Description Permanent contract | £27,711 to £32,654 per annum (Scale F/G) | 37 hours per week | St James Health Centre, Kings Lynn We have an exciting opportunity for an Assistant Practitioner to join our enthusiastic team within the West Norfolk Home First Hub Social Work Team. We work alongside a multidisciplinary team of community health colleagues and hospital discharge team. The Home First Hub primarily supports older people and adults with physical disabilities who are ready to be discharged from hospital but need some support to do so, or who need some extra support to avoid being admitted to hospital. The integrated team work together to determine discharge pathways for patients and for assessing a person’s needs who may require admission avoidance support. The organisation is working to create a robust Discharge to Assess Model which will strengthen discharge pathways and journeys for patients, so this is a really exciting time to join the team and be part of this transformation. In this role you will be required to undertake Care Act Assessments to aid hospital discharges or prevent hospital admission. Assessments will be carried out in a strengths-based approach to promote independence. You will also be developing Support Plans suitable to meet people’s needs including Direct Payments and Personal Budgets. You will also be able to consider Carers Assessments for informal carers. You will participate in the team’s duty rota to aid the duty function of the service. This is an exciting time to join an enthusiastic, supportive and committed multi-disciplinary team. Experience of undertaking assessments in social care and having knowledge of the Care Act 2014 including Safeguarding Adults is highly desirable. You must be a team player but also have the ability to work independently. You will require excellent communication and recording skills. The work is fast paced and assessments must be undertaken in a timely manner. This is a full-time position that is office based at St James’ Clinic, King’s Lynn.
