Working within START; you will be working within a team focussed on assisting Service Users to return to their homes safely from hospital with a package of care. This work is about helping people achieve the things in life that really matter to them. You’ll go home each day knowing you have made a difference.
You will provide support and assistance to Home Care Coordinators and Locality Lead Officers and help to monitor the quality of the service delivered.
You will assist with the day to day running and managing of care packages, recording, monitoring staff training, attendance management, Microsoft Teams, liaising with other professionals, assisting to facilitate packages of care transferring to long-term providers.
To ensure safety of staff and service users you will update documented Care Plans including the risk assessments and handling plans. You will ensure up to date records are kept and all care provisions meet the needs of the service user.
The Person
4. Should have IT skills and be able to embrace technology.
5. You should have the ability to work well within a team in a fast-paced environment and have excellent communication skills.
6. The ability to engage with others and form part of a multi-disciplinary team to achieve common goals and positive outcomes for service users and staff.
7. Throughout your employment you will undertake a programme of continuous professional development and be willing to undertake SVQ qualification training to a level 3 in Health & Social Care.
Before confirming your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland.
