Job summary
The post holder will work with children and young people aged 5-18 who are in the care of the local authority. The position covers the locality of Norfolk and Suffolk.
You will complete statutory health assessments with children, young people and their carers in order to support better health outcomes of children in care.

Main duties of the job
To work for East Coast Community Healthcare CIC (ECCH) in the health service for Looked After Children which provides a service across ECCH, Norfolk and Suffolk and its partner organisations in line with statutory national and local guidance and recommendations including Care Matters.

To complete statutory health assessments with children, young people and their carers for children, young people aged 5-18 who are in the care of the local authority.
To promote and improve the health of Looked After Children by providing direct services and enhancing local co-ordination and communication between young people, carers, health, education, social services and voluntary agencies.
To assess and formulate the health needs of Looked After Children and young people, including physical and emotional aspects, cultural diversity, and health-promoting activities.

The Looked After Children's team is a small team within East Community Healthcare which consists of a Service Lead, Team Manager, Clinical Lead Specialist, Nurse Specialist for Looked After Children and Admin support.

Job responsibilities
Following national and local guidelines, plan,organise, deliver, and evaluate comprehensive health assessments to assess andformulate the complex needs of a looked after child or young person usingevidence-based knowledge to develop health plans toward improving the long-termhealth outcomes for looked after children
Empower children and young people to takeresponsibility for their own health and wellbeing
The post holder needs to be an autonomous practitionerwho can develop and embrace a flexible range of working regarding the wayservices can be delivered thereby ensuring achievement of positive outcomes forLooked After Children and Young People
To work as part of a small team in further developingthe LAC Specialist service, whilst maintaining and developing relationshipswithin Health and partner agencies
Ensure that the health of Looked After Children ispositively promoted, encouraged, and supported
Work closely with all professionals involved to ensurea comprehensive health assessment for children and young people is undertaken
Develop and undertake age-appropriate health promotionadvice and act as a resource providing health advice to children, carers, andprofessionals
Establish robust lines of communication relating tohealth care between service users, professionals, and agencies
To attend, as appropriate, reviews for Looked AfterChildren to inform and advise on the child/young persons health needs
Ensure those children who are Looked After byNorfolk/Suffolk Local Authority but are placed out of County have their healthneeds met in a timely and consistent manner
To establish and maintain effective links acrossorganisational and inter-agency boundaries
To provide professional advice and training on issuesrelating to the health of Looked After Children to carers and other agencies
To model a good standard of health assessments/reportwriting for Looked After Children and to support other health staff inundertaking these
To communicate and receive highly complex and sensitiveinformation in relation to Looked After Children
Compile reports for Looked After Childrenreviews/conferences and store and retrieve these documents electronically

1. Post Registration Training in Safeguarding Children/LAC (willingness to learn)
2. Evidence of learning at Degree level
3. Good communication skills with children and young people, and with adults
4. Good knowledge of child development
5. Ability to write comprehensive health care plans, records, and reports

7. Ability to prioritise workload effectively to meet deadlines

Qualifications

14. NMC registration as registered general, childrens, mental health or learning disabilities nurse
15. Registered on Part 3 of the NMC register as a Specialist Community Public Health Practitioner (Health Visitor or School Nurse) or holds relevant equivalent experience
16. Proven evidence of continued professional development
17. Up to date and relevant knowledge of children and young peoples legislation and guidance
