About The Role

As an Advanced Practitioner, you will lead on peer development and practice, supports and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire’s Connected Lives principles to contribute to the ongoing development of the service.

Advanced Practitioners are responsible for delivering high quality statutory social care including recognising safeguarding, delivering assessment, supervision and care management functions as well as developing innovative, creative community solutions.

Advanced Practitioners are pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families, and carers.

There is responsibility for ensuring that all statutory health and social care responsibilities are met and delivered to a high standard including safeguarding, assessment, supervision, and care management functions.

We are re-energising our adult social care model of Connected Lives approach that has a strong ethos of strength-based working, identifying personal goals and ensuring personal, family and community resources are utilised to achieve positive outcomes for people.

Our teams

Connected Lives Gateway Connections - East

We have an opportunity for a Social Work Advanced Practitioner to join our Connected Lives Gateway Service who are the front door to adult social care in Hertfordshire and aims to prevent, reduce and delay need by ensuring people can access a wide range of information, advice and support. We’re making a significant investment in prevention, offering a unique opportunity to shape services that deliver better outcomes for people.

This multi-disciplinary team of social workers, occupational therapists and community care officers works flexibly, both from the office and remote locations, to bring together a vast amount of experience and skill in providing initial assessments and tailored information to new people approaching adult social care. You would be working alongside a variety of partners to achieve the best preventative solutions.

Older People Team

We have vacancies within:


* Hertsmere Team - Fixed term to January 2026


The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We have a range of teams within Older People services, including locality based teams, both residential and community reviewing teams, crisis intervention and prevention of hospital admission teams, and sensory services. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.


* Integrated Discharge Team - West


We have opportunities for a Social Work Advanced Practitioner within the following teams:


* Watford General Hospital
* Post Hospital Review Team
* Integrated Discharge Team Advanced Practitioners currently receives a non-contractual market enhanced payment and a 5% operational payment. Your contracted salary will be £46,731 - £51,802 pa.


We provide a 7-day service and currently operate a 1-weekend-in-8 rota across the Hospital Team to improve outcomes for service users. You'll be part of a service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends.

You may be required to visit service users in the community to undertake reviews, so a driving licence would be essential as is the ability to respond to situations quickly and flexibly.

Other Advanced Practitioner vacancies may arise in other teams, therefore we encourage you to submit an application if you meet the criteria, to be considered for future vacancies and we will contact you.

About You

You’ll be a qualified social worker or occupational therapist with a minimum of 2 years post qualifying experience.

In addition, you will also need the following: Actively undertakes a specialist role i.e. Moving and Handling Practitioner, Practice Educator, Approved Mental Health Practitioner, Best Interest Assessor - or agreement to undertake a specialist role within the first year of the role.

You will have extensive knowledge and experience in the field of adult social care, with an emphasis on safeguarding practice and related legislation. You will have the ability to manage competing service priorities and a sound grasp of current legislation, personalisation agenda and meeting the business objectives of the department. Demonstrating management potential, you will have excellent all-round communication and negotiation skills working with the Statutory, Voluntary and Independent sector partners.

You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support. You will utilise your knowledge of the Care Act 2014, Mental Capacity Act 2005, and other social care legislation, to manage the most complex cases. You will use of your advanced knowledge and experience to support other members of the team and the management team to ensure good practice. You will develop close working relationships with hospital staff and care providers to ensure effective care planning and balance the use of resources.

The vacancy within the Hertsmere Team - Fixed term to January 2026 is open to secondments. If you work for HCC or any other Local Authority and would like to be considered as a Secondment contract, please ensure you complete the Secondment Form as part of your application.
