Overview
An exciting opportunity has arisen for a Band 6 Social Worker to join the Manchester Adult Cystic Fibrosis (CF) Team, located at Wythenshawe Hospital, and part of the Manchester University NHS Foundation Trust (MFT).
The Manchester Adult Cystic Fibrosis Centre (MACFC) is a specialised regional centre that provides personalised and holistic care and support to over 480 individuals with CF who live in the Northwest.
You will be a core member of the CF Multidisciplinary Team (MDT), collaborating closely with clinical psychologists, specialist nurses, dietitians, physiotherapists, occupational therapists, and medical staff. We are looking for someone self-motivated, dynamic and passionate about delivering excellent psychosocial care to adults with CF. You will need to be flexible, a team player, and able to demonstrate a commitment to professional development and evidence-based practice.
You will provide proactive social work support to individuals with CF, and their families where necessary, around a range of psychosocial issues, including access to resources, community care, employment or education, housing, end of life care and bereavement, as well as supporting those with additional vulnerabilities such as learning disability, mental health, substance misuse or other issues.

The successful candidate will work 25 hours over 3 days with the aim of:
Providing high-quality social work support to individuals and families in inpatient, outpatient, and community settings. Assessing the psychosocial conditions of individuals with CF and their families and connecting them with resources to meet their needs. Offering professional social work guidance on social work and psychosocial issues to the wider CF Team.
