Overview

Cheshire and Wirral Partnership NHS Foundation Trust (CWP) operates a range of health and care services across the region, including mental health, learning disability and community physical health. CWP is rated Outstanding for Caring by the Care Quality Commission and holds a Good overall rating.

Healthier South Wirral Primary Care Network (PCN) and CWP’s Children, Young People and Families mental health services (CYPMHS) work together to enhance access to mental health support for children and young people in South Wirral.

We aim to deliver an integrated community‑based model of mental health care supporting both primary and secondary services and to facilitate the right help in the right place at the right time.

Healthier South Wirral Care Network is a collaboration of six GP practices – Sunlight Group Practice, Spital Surgery, The Orchard Surgery, The Allport Surgery, Eastham Group Practice and Civic Medical Centre – serving over 49,000 patients.


Main duties of the job

The role involves completing assessments and delivering brief interventions for patients registered at the GP practice who have mental health needs, or signposting to other provision in school or the community. The practitioner will support access to the graduated approach for potential neuro‑developmental needs, signpost following assessment by secondary CYP Mental Health Services (CYPMHS), facilitate transitions out of CYPMHS, and build community networks while offering training and upskilling. Expert advice and support will also be provided to enable the GP MDT to deliver effective care for children and young people with mental health needs.

The practitioner will be line‑managed and professionally supervised by the CWP Band 7 Access Team Manager and will receive training, education and support from CWP.

Key relationships will include Branch Matching Panel, Community CYPMHS teams, LD CYPMHS teams, 3rd Sector pathway partners and Community Paediatrics.


Person Specification


Qualifications

* Registered Mental Health Professional with a relevant professional qualification (e.g. RMN, Occupational Therapist)
* Educated to degree level in a relevant field or equivalent experience
* Master’s level qualification or equivalent post‑qualifying short courses/experience
* Evidence of recent post‑qualifying continuing professional development
* Teaching qualification or significant teaching experience
* Post‑graduate psychological therapy qualification or equivalent


Knowledge and Expertise

* Excellent communication skills – working with staff, patients and carers, general practice, presentations, written reports, and facilitating evidence‑based practice
* Ability to challenge current practices and implement change based on evidence‑based recommendations
* Understanding and implementation of local and national policy direction relevant to practice
* Experience of clinical audit and facilitating staff participation in audit
* Empowering staff to manage and support change
* Effective clinical leadership with knowledge of leadership theories
* Clinical expertise to support and facilitate development and innovation
* Sound information technology skills


Experience

* Acting as a clinical supervisor
* Developing professional roles
* Managing projects that demonstrate communication and change implementation
* Understanding of the clinical and corporate governance agenda
* Facilitating reflective practice for staff and personal reflective practice
* Managing complex, sensitive or contentious information in emotive situations
* Direct delivery of a clinical, technical or social care service
* Delivering specialist programmes of care or care packages
* Motivating and empowering others to change roles and leading practice development
