This exciting role is in the Gravesend Alliance/Springfield Health PCN (Gravesend locality). The role helps to bridge the gap between Primary Care and specialist Mental Health services by delivering high quality Mental Health care in Primary Care settings and ensuring people\'s needs are met in a timely way.
The position requires travel between various GP surgeries in the locality in a timely manner. You will be a highly competent, proficient practitioner who can work autonomously and creatively in this busy role. Excellent assessment skills and knowledge of local services are essential. Leadership experience is valuable, as you will provide supervision to Band 6 Mental Health Practitioners across the locality.
Successful candidates must be experienced Mental Health Practitioners with experience in a Primary Care and/or Community Mental Health setting. Roles are open to all professions. Responsibilities
* Bridge the gap between Primary Care and specialist Mental Health services by delivering high quality mental health care in primary care settings.
* Travel between GP surgeries within the Gravesend locality to deliver timely care.
* Work autonomously and creatively in a busy primary care environment.
* Carry out comprehensive assessments and coordinate care with local services.
* Provide supervision to Band 6 Mental Health Practitioners across the locality.
Qualifications and Skills
* Experience as a Mental Health Practitioner with a background in Primary Care and/or Community Mental Health settings.
* Ability to work autonomously and as part of a multidisciplinary team.
* Leadership experience and strong supervisory skills are valuable.
* Excellent communication and stakeholder engagement abilities.
* Open to all professional backgrounds in mental health care.
