Overview
MH Senior Health Care Support Worker - First Transitions Team at Midlands Partnership NHS Foundation Trust. This is a Band 4 Health Care Support Worker role within Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting the First Transitions Pathway.
The First Transitions Pathway is a new service to help support individuals with complex mental health needs as they move from forensic services to community-focused care. This transformation aims to promote sustained recovery, meaningful community integration, and empowerment for service users.
Main duties
You will be instrumental in providing practical and emotional support directly to service users during the transition phase and in the weeks and months that follow. Responsibilities include building rapport, applying engagement strategies, and facilitating access to community resources.
* Provide intensive and assertive outreach to service users at risk of disengagement, aiming to prevent relapse.
* Provide practical support within community settings, addressing immediate needs and implementing engagement strategies.
* Be curious and motivated to make a difference to this cohort of individuals.
Qualifications
Essential
* Foundation degree in relevant subject or Diploma/NVQ 3 in Health and Social Care or accredited prior learning with competency-based evidence supplemented by short courses.
Desirable
* Training in working with adults/older adults with complex mental health needs and/or challenging behaviour, and/or other behavioural work.
Experience
Essential
* Significant experience of working with adults with mental health needs.
* Experience of working within a multi-agency framework.
* Experience of working in a forensic setting.
Desirable
* Previous work with people with complex and high-risk presentations.
* Significant experience of supporting service users in crisis home-based/community settings.
