What the Job Involves The role involves providing direct social work services to adults and families identified as in need of care, working in partnership with colleagues across sectors to ensure an integrated response to meet these needs. You will engage directly with individuals and their families to ensure their participation in decision-making, assessments, and care planning. The role includes conducting safeguarding investigations, supporting care placement arrangements, and promoting independence. In addition, you will maintain accurate records, prepare and present reports, and actively contribute to professional development and service improvement initiatives. Supporting integrated practice, you will chair multidisciplinary meetings and act as a role model for professional conduct and learning.
