Overview
Advanced Nurse Practitioner (30 hours per week) at Welton Family Health Centre.
This is a full-time position (30 hours per week, Monday to Friday). Responsibilities include consulting with adults and children, some triaging of patient requests via our online consultation tool, conducting home visits as needed, and managing minor illnesses and chronic conditions. Candidates should be confident in history taking, assessment, examination, and diagnosis, demonstrating evidence-based clinical decision-making and critical thinking.

Main duties
* Condu ct comprehensive physical examinations, telephone assessments and clinical assessments.
* Contribute to triage of clinical requests, helping manage demand efficiently.
* Diagnose and manage a wide range of acute and chronic health conditions.
* Prescribe medications within scope of practice and relevant prescribing authority.
* Order and interpret diagnostic tests such as blood work, X-rays, ECGs, etc.
* Initiate and manage care plans, including referrals to secondary or specialist care.
* Provide health education and counselling to patients and their families.

Job Description – Key Responsibilities
Clinical Practice

* Practice independently under national and local protocols, with access to medical support when required.
* Exercise professional judgment in complex and unpredictable situations.
Leadership & Collaboration
* Lead and support clinical decision-making within the multidisciplinary team.
* Contribute to service development and improvement initiatives.
* Mentor and support junior staff, nurses, and students.
* Participate in audits, research, and evidence-based practice to improve patient outcomes.
* Collaborate with the wider practice team to meet patient needs and support practice policies and procedures.
Professional Development
* Maintain clinical competence through continued professional development (CPD).
* Participate in appraisal and revalidation processes.
* Stay up to date with relevant legislation, clinical guidelines, and best practices.
Qualifications & Requirements
Experience
* Minimum of 35 years of post-registration nursing experience.
* Proven experience in a relevant clinical setting (e.g., primary care, acute care, mental health).
* Demonstrated ability to manage complex caseloads autonomously.
Qualifications
* Registered Nurse with active registration - NMC in the UK, or relevant board.
* Masters degree in Advanced Clinical Practice or equivalent.
* Independent Non-Medical Prescriber qualification - V300 or equivalent.
