Admission Avoidance Team
We are seeking to appoint a dynamic, confident, innovative and flexible Nurse or AHP with excellent communication and organisational skills. The post is 37.5 hours per week and is a permanent position to support the delivery of urgent community response and hospital at home/virtual wards.
This post holder will join our existing team of high functioning, highly motivated Advanced Clinical Practitioners, who deliver a wide variety of services including 2 hour urgent community response, hospital at home/virtual ward, falls pick up services and dedicated care home support, all enable people to be cared for in their usual place of residence. The Advanced Clinical Practitioners operate seven days a week between 08.00 to 20.00.
The post holder will work with partners system wide to provide integrated care across the health economy. The applicant will have qualified and hold an MSC in Advanced Clinical Practice.
The applicant must be able to travel across the locality independently in accordance with the Equality Act 2010.
Responsibilities
* Deliver 2 hour urgent community response
* Provide hospital at home/virtual ward support
* Offer falls pickup services and dedicated care home support
* Enable people to be cared for in their usual place of residence
* Operate seven days a week between 08:00 and 20:00

Qualifications
* Registered nurse or Allied Health Professional (AHP)
* MSc in Advanced Clinical Practice
* Ability to travel across the locality independently in accordance with the Equality Act 2010
Additional information
New Health and Social Care Act Regulations were approved by Parliament on 22 July 2021, which requires all those working or entering a CQC registered care home (for persons who require nursing or personal care) to be fully vaccinated (Covid-19) from 11 November 2021, unless exempt under the regulations. You will therefore be required to provide proof of vaccination or exemption if you are successful at interview. Health information is classed as special category data under UK GDPR legislation therefore, vaccination/exemption confirmation will be processed and stored in line with Data Protection Act 2018.
