Key Responsibilities:
* Conduct assessments of service users’ needs and formulate personalised care plans.
* Offer support and guidance to service users, their families, and caregivers.
* Collaborate with various professionals and agencies to ensure comprehensive support for service users.
* Regularly review and update care plans to accommodate changing needs and circumstances.
* Advocate for service users, ensuring their rights and preferences are respected.
* Operate within legal and regulatory frameworks to safeguard vulnerable adults.
* Maintain accurate records and documentation of interactions and interventions.
Requirements:
* Qualified Social Worker with a relevant degree and registration with Social Work England.
* Demonstrated experience in adult social work, preferably in a similar environment.
* Proficient assessment and analytical skills, capable of effectively identifying needs and risks.
* Excellent communication and interpersonal abilities, adept at building rapport and trust.
* Ability to collaborate effectively within a multidisciplinary team and with external partners.
* Thorough knowledge of relevant legislation, policies, and procedures concerning adult social care.
* Commitment to continuous professional development and adherence to ethical standards.
