You will be responsible for supporting the Office Team to ensure that the care we deliver is of the highest standard.
Job Description
The role is to support the delivery of care services by carrying out a range of functions in the field, including the assessment and review of individual client services and the practical supervision, instruction, and support of our carers.
The key aspects of the role will include:
1. Providing full support to our team of Care Givers.
2. Working closely with the Care Manager to coordinate care services and ensure the best Care Giver and client match.
3. Liaising with family members and relevant healthcare professionals regarding client care.
4. Assisting in planning, coordinating, and scheduling client support to ensure continuity of service.
5. Assisting with Care Giver training activities.
6. Managing daily medication and task prompts.
7. Following up on alerts to ensure timely service delivery.
8. Managing and recording Care Giver annual leave requests.
9. Assisting with Care Giver recruitment.
10. Addressing queries promptly and appropriately, referring to relevant team members as needed.
11. Building and maintaining effective communication with clients, relatives, Care Professionals, and other professionals.
12. Writing care plans for new clients.
13. Supporting local community and business events and activities.
14. Ensuring compliance with quality assurance standards in line with company procedures and CQC regulations.
15. Reporting recruitment needs to fulfill client requirements.
Our Care Coordinator will split their working time equally between community office and field activities.
