Overview
Integrated Care Worker (Rehab) – development role. Permanent contract in Cambridge. The Home First Intermediate Care Service supports patients to achieve independence by enabling earlier discharge from hospital and avoiding hospital admission. You will assist in delivering high standards of health care and support patients in their own homes by following prescribed packages of care and therapy.
Our vision is outstanding, patient-focused, community-based health care and therapy that supports earlier discharge, with early assessment for care needs and avoidance of hospital admission. Our operating hours are 7am to 10pm, 7 days a week.
Responsibilities
* With training, use skills from nursing, allied health professionals and medicines management to undertake delegated tasks.
* Apply competency-based treatment techniques and care to patients under the supervision of a qualified practitioner, following a prescribed plan.
* Provide care to patients with deterioration in health or function with a focus on admission avoidance to acute hospitals.
* Deliver functional assessments, personal care, medication prompts, mobility support, appropriate exercise, personal and domestic activities of daily living, and equipment provision.
* Carry out duties delegated and supervised by registered professionals.
* Provide a high standard of care and manage a caseload of patients with ongoing complex needs.
Requirements
* Travel independently around the county meeting strict time deadlines; full UK driving licence and use of a vehicle required.
* Role may be offered as a Band 2 Development post depending on qualifications and experience, with support to develop into a Band 3.
