We are recruiting two Health and Social Care Assistants to join our community care team. These posts are essential in meeting service needs across early and late shifts, supporting people in their own homes to maintain dignity, wellbeing and independence. Flexibility is key, as the service runs across two shift patterns.
Main duties of the job
What you’ll do:
* Provide personal care and practical daily support in line with individual care plans
* Support with meals, hydration and taking prescribed medication
* Observe and report changes in wellbeing to the wider team
* Keep clear, accurate records using electronic systems
* Work flexibly across early and late shifts to meet service needs
What we’re looking for:
* A caring, respectful approach and good communication skills
* Experience in care or transferable skills, with a willingness to learn
* Ability to work both shift patterns: 8am–4pm and 2pm–10pm
* Availability to work 30 hours per week across five shifts, depending on service need

Qualifications
* Level 2 in Health and Social Care or equivalent level of experience in Health and/or Social Care
* Care Certificate or willingness to complete this within a 6 month induction period
Knowledge and Experience
* Experience of working in a health and/ or care environment
* Experience of prioritising own work load
* Good understanding of basic IT skills
