Do you want to help patients remain living independently in their own homes? Would you like to work within a multi-disciplinary team? If so, we have an exciting opportunity for a dynamic registered specialist Nurse or Physiotherapist to join our highly specialised respiratory team, providing care to patients with respiratory disease across North of Tees.
About the role
The post holder will be responsible for delivering high-quality care to patients in Hartlepool and Stockton, in both clinic and home settings. You will manage patients proactively, requiring immediate specialist assessment and pathway-based interventions, with direct links to hospital and support from a wider team, including consultants.
This role involves assessing and treating patients with complex presentations or multiple pathologies using advanced assessment techniques and adapting care plans according to patient needs. Flexibility in working hours, including weekends, is required. Both full-time and part-time hours will be considered. A full driving license is essential.
