Responsibilities
* Contribute to the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics by assessing, planning, implementing and evaluating packages of care.
* Facilitate/support newly qualified nurses in their Preceptorship year.
* Assess, prescribe and deliver care packages for patients, including long-term conditions, palliative care and rehabilitation needs, to improve quality of life and independence where possible.
* Work within the integrated team to facilitate early discharge from hospital and to prevent unnecessary admissions.
* Collaborate with all health care professionals and statutory/non-statutory agencies to provide a seamless, integrated service to service users.
* Support and develop junior colleagues in practice and professional development as required.
Qualifications and Skills
* We are looking for a dynamic, forward-thinking registered nurse with strong communication, leadership and organisational skills.
* Experience in supporting more junior staff and enhancing team practice is desirable.
* In-role training and development are available within a well-established team; candidates without certain skills but with leadership potential will be supported to develop.
