Overview
Residents are at the heart of everything we do. You are responsible for providing leadership and support to the coordination team and care staff. You will manage the quality and continuity of care coordination within the community whilst ensuring that an effective service is running at all times. This role will require you to be on call on a rota based system.
Qualifications
Youre a compassionate, can‑do person with experience of leading teams in a similar environment. You have already got a Level 2 vocational qualification in Health and Social Care (either a diploma or NVQ) and are now ambitious to progress further. You are good at using your initiative and working well under pressure. To you, its more than just a job; its a role where you happily go the extra mile to make a difference.
