To accurately review, summarise, and input clinical information frompatient medical records, ensuring that notes are clear, concise, and accessibleto support effective patient care and clinical decision-making.
Main duties of the job
* Review and clean up patient medical records byensuring that the problem list is accurate and up to date, removing duplicateor irrelevant entries, and streamlining notes to improve clarity and usabilityfor cliniciansEnsure that only relevant and active clinicalproblems appear on the problem list, supporting effective patient care anddecision-makingEnsure summaries are accurate, clear, andcompliant with clinical and data governance standardsAssist with backlog clearance and ongoingsummarisation tasks to support clinicians and improve record qualityTo be able to add bulk childhood immunisationsand seasonal vaccines onto the clinical systemTo be able to undertake audits of work toensure compliance and support quality improvement projects
* Identify and flag any discrepancies,safeguarding concerns, or urgent clinical issues to appropriate staff promptly

Qualifications
* Medical Terminology Training
Experience
* Experience of working with clinical records or medical documentation, ideally within primary care or healthcare.
* Excellent attention to detail and accuracy in summarising clinical information.
* Good IT skills, including experience with electronic patient record systems (EMIS preferred).
* Understanding of medical terminology and clinical language.
* Ability to maintain confidentiality and handle sensitive information appropriately.
* Strong organisational skills and ability to manage workload effectively.
* Good knowledge of data regulations
