Their fostering and permanence services focus on consistency, independence planning, and ensuring young people remain connected to the families and carers who have supported them. The Role This role is a specialist ‘Staying Put’ Fostering Social Worker position, responsible for supporting care planning for young people in foster care from age 16 onwards. The Advanced Practitioner will track progress, ensure timely information sharing, and work closely with foster carers to achieve Staying Put arrangements when the young person turns 18. Following the transition from fostering to Staying Put, the practitioner will continue to support the former foster carer (now Staying Put host), undertaking 12‑weekly visits to provide guidance, promote placement stability, and strengthen long‑term outcomes for the young person. This role is central to driving quality, performance, and oversight within the fostering service, ensuring independence planning is robust, timely, and aligned with the young person’s needs. Key Responsibilities Support care planning for young people in foster care from age 16 onwards. Track progress and ensure timely information sharing with foster carers. Facilitate and support the transition to Staying Put arrangements at age 18. Provide ongoing support and advice to Staying Put hosts through 12‑weekly visits. Promote placement stability, independence planning, and long‑term permanence. Challenge drift and delay in care planning where required. Maintain accurate case records and contribute to performance improvement. Work collaboratively with internal teams and external partners to ensure high‑quality outcomes. Candidate Requirements Qualified Social Worker with Social Work England registration. Experienced Fostering Social Worker with detailed understanding of Staying Put arrangements. Strong background in care planning and supporting post‑18 placement stability. Confident in challenging drift, delay, and barriers to progress. Skilled in relationship‑based practice with carers, young people, and professionals. Knowledge of relevant legislation, guidance, and best practice. Must be DfE compliant. Enhanced DBS required.
